How to Refer:
Referral form can be found on ARDENS (Herts P1: Community Services - 'Frailty Clinic Referral'), then to be sent to hct.communityfrailtyservice@nhs.net
Requirements Prior to Referral:
Rockwood Clinical Frailty Score of 6: Moderately Frail: People need help with all outside activities and with keeping house. Inside, they often have problems with stairs and need help with bathing and might need minimal assistance (cuing, standby) with dressing
Blood tests: FBC, U&Es, LFTs, Bone Profile, TFTs, Ferritin) - Can be within the previous three months if no acute issue
ECG if clinically relevant (e.g. blackouts, cardiac concern or arrhythmia)
Additional Information Required:
Recent A&E attendances
Past medical history
Reason for referral (as an SBAR)
Early Memory Diagnosis and Support Service (EM
